Doctors that listen, Care

The current medical system has set itself up in a way to minimize the amount of time a physician spends with a patient. It is a simple concept. Too many sick people, too many cases, and not enough time for doctors to cover them all.

Unfortunately this has led our health status into a melting pot of undiagnosed, misdiagnosed, over tested, under analyzed patients. So many physicians do not have time to spend taking a thorough history, so instead of listening to the concerns and complaints of a patient, and going through all of the symptomatology that the patient presents with, it is common to order an excessive amount of unnecessary tests to come up with a diagnosis.

While attending Palmer College of Chiropractic, I learned the most valuable thing about being a physician.

80% of our diagnosis should come from the history of the patient. We need to speak to patients for as long as it takes to come up with a very concise decision that supports the indication to do additional testing.

If you look back at the top of this article, you will see that the very definition of a symptom explains it perfectly. Symptoms are a SUBJECTIVE indication of a disease or dysfunction. Listen to what the patient is saying, but more importantly understand how the body works so you can utilize the information that is given during a thorough history.

The process of coming up with an accurate diagnosis of a patient starts with a physician’s understanding of the human body. There is absolutely no cookie cutter flow chart to be followed that will work on every single patient. Certain dysfunctions can be expressed differently in every patient, but you have to dig deeper.

When a patient discusses a symptom, it is just the tip of the iceberg. You have to know everything about that symptom. You have to know which questions to ask and how to interpret the responses. 

Lets go through a scenario:

A 39 year old female patient presents with mid to low back pain.
Symptom: back pain.

Some physicians will immediately lean toward an X-ray to rule out things. Some might automatically prescribe a muscle relaxer. NSAIDs are always suggested. Maybe ice, massage, or even electric stimulation.

Chiropractors are famous for immediately linking to a spinal subluxation and an adjustment will fix it up.

What questions should be asked?

Location of the back pain? How long has it been going on? Does anything relieve it or make it worse? All physicians understand the whole OPPQRST bit, but do they use it?

Is a kidney exam indicated? Gall Bladder stones? Lumbar disc derangement? Did they fall and break a rib? Diverticulitis?

The point I am trying to make is this; a symptom as simple as back pain can have many different diagnoses. There is any amount of testing that you can order to search for the correct condition. The true physician will minimize the cost to a patient by ruling out some of the differential diagnoses, and minimize any testing required.

Pain is definitely the most common symptom that encourages a person to seek medical attention. Low back pain being number one in the US.

The symptom of pain seems to be the one hardest to deal with, the hardest for a patient to overlook, but there are so many more symptoms that must be analyzed by a physician that is seeking a true diagnosis. Symptoms like, restless at night, weight gain, low body temperature, elevated pulse, frequent urination, and so many more are how the body expresses its state of health.

The body will tell a physician a story. They just have to listen to it. More importantly, they must not look at any patient with a predetermined mentality of what is wrong. This is how a misdiagnosis happens.

The most common complaint that I hear from patients is that their primary physician doesn’t really listen to them.

Has this ever happened to you?

It is probably not the physician’s fault. They work in a health system with too many sick people, and unfortunately, testing is profitable and major hospitals are a business. All business’s like making profit, so the more testing they order, the more money they make.

I like to be a cost effective physician, but even more so, I like to be accurate. It is not hard to come up with a diagnosis if you just listen, and ask the right questions. I order tests when needed. I order imaging if necessary. They are NEVER my primary source of coming up with a diagnosis. I run at about a 90% rate of using diagnostic testing to confirm what I already determined from a thorough history. I only test if my safe, non-invasive treatment plan is not providing the results I think the patient deserves. Physicians have to start being fair, and not going beyond their expertise.

What can spending $10 billion a year in advertising sell?


Have you ever wondered what it would cost to completely skew the minds of the masses in America and sell a product that is not only proven not to work, but proven to make you worse? They defend something that has taken years to ingrain into each of our brains that this product is necessary, against all odds, against all real research, the product must be good because doctors prescribe it and celebrities use it.

These companies have mastered the art of selling you and me products to help heart burn, lower cholesterol, help the common cold, and vaccinate us from the years flu bug.

But how has this marketing strategy really affected us?
Whether or not you know what Psoriatic Arthritis is, or how it affects the body, we, as Americans, all know that Phil Mickelson suffers from it. We know that he is a professional golfer that has a condition that can only be helped by taking a very specific medication and if he doesn’t take it, he will never be able to play golf again.

This advertisement is designed to put a personal touch on a celebrity or professional athlete and help each middle age American male think about if their pain that they are suffering from can be treated with this MIRACLE medication called Embrel. So we all tread off to the overworked, understaffed, ever too busy medical doctor and ask if this will help. All the doctor has to do is ask if you have pain, and when you say yes, you are linking yourself to the symptoms that Phil suffers from on the television, and you are not there for anything other than the same medication he uses. This means you are already a customer to this Pharmaceutical Company, and you are a lifetime buyer of their product. This is successful marketing, but at what cost???

Always pay attention to the “fine print”

We have all been taught to analyze the “fine print”. We know that in the fine print you will find things about any product that you would not like. Like the interest rate on a credit card. It is not in big bold print on the front, unless it’s a 6 month promotion of 0%APR, but the fine print will say something like, if you don’t meet your minimum payment or any carried credit balance will be subject to 21.99%APR after the 6 month grace period. So to be clear, we all know that the “fine print” of any product give details that the provider does not want you to focus on, and they create a diversion to keep their perspective client focused on the “good” of the product and not pay attention to the “bad”.

When you have $10 billion to spend on marketing, you can make sure that each one of your ads are professionally designed and ran through many control trials to analyze their effectiveness to the consumer before they ever hit the public eye. The entire ad is designed to get your attention, make you feel like you or someone you care about is suffering from the condition they are trying to exploit, and then show you the only way it can be treated, their miracle medication. After the commercial gets you all warm and fuzzy inside, and you have already self-diagnosed yourself, all you have to do is get your primary care physician already on the phone to schedule an appointment to get your prescription, then they lay the “fine print” on you. This comes in a form of extremely fast, monotone script of the many side effects that come along with the minute benefits of this miracle drug ,but you are already so warmed up from the well-designed commercial that you don’t even make an effort to pay attention.

Side effects like increased chance of infection, rapid pulse, increased urge to gamble, drowsiness, etc do not mean anything to us. We are not worried about what can happen, we are only focused on what we want to happen, and these pharmaceutical ads are genius at telling us that they can do just that.
I am not writing this blog to solely reiterate what we all know about the dangerous side effects that come with all medications, the fact that they exist, and the fact that we all ignore them. I am writing to tell you about how spending billions of dollars on advertising can sell anything, even things that can harm your health by trying to help it.

Have you ever wonder how much it cost to make a medication?

Would it alarm you if you know that a study shows that pharmaceutical companies have reached a level of domestic sales to a number of $235.4 billion, in the US alone. Research shows that 13.4% of this amount is spent on research and development of new medications.  To do the math for you, about $32 billion is spent on research and development of new medications.

How much is spent on marketing their products?

The same research paper states that pharmaceutical companies spend 24.4% of their sales on promotions of their products. This is how pharmaceutical industries spend $57.4 billion dollars on marketing. About $10 billion goes to direct marketing to consumers in the form of TV ads, billboards, and celebrity promotions. The rest is spent on Pharmaceutical Reps, Salespeople, doctor’s lunches etc.
With this volume of advertising, we can not help but become clients. It is all we see, all day, every day. A pill for this, shot for that.

Do you think it is odd, that if you get a flu shot at CVS, they will give you 20% off your store purchases that day?

I am a well-researched anti flu shot physician. I know all of the harmful side effects, and lack of scientific evidence that it is even remotely effective to prevent the flu, and I almost went and got a flu shot to get the 20% off.
The biggest industries have the most power and influence over us.  We need to stop buying into the system, and start a proactive way of living to promote our own health and stop being pawns in the medical scheme.

Fibromyalgia, Scoliosis, GERD, etc.One Cause. A Chiropractor Can Help

This term refers to a condition called brain laterality, meaning one side of the brain’s cortex is over stimulated in regards to the other side. The most common form of brain hemisphericity is right brain dominance. The left and right brain are associated with two major hormones in the body, testosterone and estrogen respectively. The female sex is more susceptible to being effected by emotion or fear is because estrogen and the right brain are associated with emotional control. When the brain is constantly stimulated by emotion, it activates the right cortex, causing an overstimulation and leads to right brain dominance.

Other causes of brain laterality are lack of activation of the cerebellum. Cerebellum is the part of the brain that is responsible for our motion detection and balance. If there are fixated joint segments on the left side of the body, the cerebellum will not receive any motion information, leading to a decrease in cerebellar function. The trick is, that all sensory information that comes from the body, goes to the opposite side of the brain. So any lack of motion in a joint segment on the left side of the body, will shut down activation on the right cerebellum. This can lead to activation of the right cortex, leading to right brain dominance.

Muscle control is also stimulated by the opposite side of the brain, so your left arm is controlled by the right cortex. This is important because when a person falls into right brain dominance, the muscles on the left side of the body are over activated. This doesn’t affect every muscle in the body, mainly just the postural muscles that are fired automatically, meaning they fire without any of our control. The majority of postural muscles are the ones that support the spine. When the body becomes right brain dominant, there is an over activation of the left side spinal muscles in regards to the right side, leading to idiopathic spinal scoliosis. This is why the spinal curvature is always to the left.

Prolonged durations of right brain dominance, brain hemisphericity, brain laterality, can lead to many more common symptoms. As the right brain becomes over stimulated, it starts to build more neuronal ties to the brain’s limbic system. The limbic system is what controls our response to a stimulus, and controls our sympathetic nervous system. The sympathetic nervous system is responsible for our Flight or Fight response to a stimulus. When we over activate the sympathetic nervous system, we shut down the parasympathetic nervous system.

The parasympathetic nervous system is responsible for our digestive function. When you over stimulate the sympathetic nervous system, and shut down the parasympathetic nervous system, you inhibit the body’s ability to digest food, leading to number of conditions, IBS, GERD, ulcers, diverticulitis, Leaky Gut, Crohn’s, and even colon cancer. Poor digestion also leads to a deficiency in nutrients, most importantly, B-vitamins. Deficiencies in B-Vitamins can be linked to fibromyalgia, restless leg syndrome, infertility, depression, chronic fatigue, and even insomnia.

Common signs and symptoms of right brain dominance is someone who uses feelings as a way of describing things, someone who has a big imagination, someone that draws and is more interested in art than math. 

This can be treated, and alleviated with controlled care. Stimulation of the nervous system on the left side of the body will increase cerebellar function on the right side of the body, inhibiting the right cortex from being over stimulated. Also, lifestyle changes, that allow us to cope with stress with also decrease the emotional stimulation of the right cortex. This therapy takes time, but is very effective.
