3 Years and Some Change

Upon graduating for Palmer College of Chiropractic, Florida in December 2011, I set out to create a health center that bases its principles around changing what you think and know about the medical system. With a strong grasp on Evidence-Based Chiropractic Care and an evolving knowledge on nutritional therapy, tying the two holistic medicine approaches together would be the premise of New-Start Health Center.

Opening a business that is designed to go against the grain is never easy. Fresh out of college, I definitely felt on my own. There are many chiropractic management companies that are designed to help young businesses get off the ground, but I wanted to pioneer something unique. The primary disconnect for myself and chiropractic management companies is the overall feeling of chiropractic in our society. We have all heard the horror stories of going to a chiropractor, “once you go you have to go forever” or “they spend 2 minutes with you and just crack your back”. Trying to change the mind of the majority on how New-Start is a different style of chiropractic. Evidence-Based Chiropractic does not X-ray every patient. We treat pain syndromes and we release them from care. I spend about 12-15 minutes with every patient, minimum. This is a dramatic change to the stereotypical chiropractic visit.

As I stated, an evolving knowledge on nutritional therapy, I have spent countless hours in a biochemistry book, in seminars, and scouring the internet for research studies to figure out exactly what is going wrong. The common medical practices for diabetes and obesity have fallen short. We are getting sicker, fatter, and unhealthier at a faster rate than ever previously recorded. Unfortunately, healthcare costs are also at an all-time high and climbing quickly. In the anecdotal world, the treatment of conditions starts with covering up the symptoms, and with low back pain, or migraines, this is not detrimental. When you start to use this approach toward an inflammatory condition like Diabetes or Obesity, more problems arise.

After years of research it seems, 5 or 6 book starters, I am proud to be writing a book called, How Health Comes Apart. The idea behind this book is the primary focus of the nutritional therapy side of New-Start Health Center. I am writing my research to change America’s way of thinking about our obesity epidemic.


In three years, New-Start Health Center has been all about change. Change how chiropractic is viewed and utilized. Change how we diagnose and treat obesity and diabetes, and change how medical systems define success. The first three years in the Columbus area have been encouraging, insightful, and rewarding.  I am proud to be a part of the maturity of New-Start Health Center, and with many more years to come, to continue to change for the better.

Two of the top three conditions that drive us into a medical office are joint pains, and back problems. Typical medical practices for these conditions are:

As you can see, it is not hard to understand how effective and life changing chiropractic care can be. It is just a hidden secret to most.

Doctors that listen, Care

The current medical system has set itself up in a way to minimize the amount of time a physician spends with a patient. It is a simple concept. Too many sick people, too many cases, and not enough time for doctors to cover them all.

Unfortunately this has led our health status into a melting pot of undiagnosed, misdiagnosed, over tested, under analyzed patients. So many physicians do not have time to spend taking a thorough history, so instead of listening to the concerns and complaints of a patient, and going through all of the symptomatology that the patient presents with, it is common to order an excessive amount of unnecessary tests to come up with a diagnosis.

While attending Palmer College of Chiropractic, I learned the most valuable thing about being a physician.

80% of our diagnosis should come from the history of the patient. We need to speak to patients for as long as it takes to come up with a very concise decision that supports the indication to do additional testing.

If you look back at the top of this article, you will see that the very definition of a symptom explains it perfectly. Symptoms are a SUBJECTIVE indication of a disease or dysfunction. Listen to what the patient is saying, but more importantly understand how the body works so you can utilize the information that is given during a thorough history.

The process of coming up with an accurate diagnosis of a patient starts with a physician’s understanding of the human body. There is absolutely no cookie cutter flow chart to be followed that will work on every single patient. Certain dysfunctions can be expressed differently in every patient, but you have to dig deeper.

When a patient discusses a symptom, it is just the tip of the iceberg. You have to know everything about that symptom. You have to know which questions to ask and how to interpret the responses. 

Lets go through a scenario:

A 39 year old female patient presents with mid to low back pain.
Symptom: back pain.

Some physicians will immediately lean toward an X-ray to rule out things. Some might automatically prescribe a muscle relaxer. NSAIDs are always suggested. Maybe ice, massage, or even electric stimulation.

Chiropractors are famous for immediately linking to a spinal subluxation and an adjustment will fix it up.

What questions should be asked?

Location of the back pain? How long has it been going on? Does anything relieve it or make it worse? All physicians understand the whole OPPQRST bit, but do they use it?

Is a kidney exam indicated? Gall Bladder stones? Lumbar disc derangement? Did they fall and break a rib? Diverticulitis?

The point I am trying to make is this; a symptom as simple as back pain can have many different diagnoses. There is any amount of testing that you can order to search for the correct condition. The true physician will minimize the cost to a patient by ruling out some of the differential diagnoses, and minimize any testing required.

Pain is definitely the most common symptom that encourages a person to seek medical attention. Low back pain being number one in the US.

The symptom of pain seems to be the one hardest to deal with, the hardest for a patient to overlook, but there are so many more symptoms that must be analyzed by a physician that is seeking a true diagnosis. Symptoms like, restless at night, weight gain, low body temperature, elevated pulse, frequent urination, and so many more are how the body expresses its state of health.

The body will tell a physician a story. They just have to listen to it. More importantly, they must not look at any patient with a predetermined mentality of what is wrong. This is how a misdiagnosis happens.

The most common complaint that I hear from patients is that their primary physician doesn’t really listen to them.

Has this ever happened to you?

It is probably not the physician’s fault. They work in a health system with too many sick people, and unfortunately, testing is profitable and major hospitals are a business. All business’s like making profit, so the more testing they order, the more money they make.

I like to be a cost effective physician, but even more so, I like to be accurate. It is not hard to come up with a diagnosis if you just listen, and ask the right questions. I order tests when needed. I order imaging if necessary. They are NEVER my primary source of coming up with a diagnosis. I run at about a 90% rate of using diagnostic testing to confirm what I already determined from a thorough history. I only test if my safe, non-invasive treatment plan is not providing the results I think the patient deserves. Physicians have to start being fair, and not going beyond their expertise.

5 ways to know if you are a Chiropractic Patient or Chiropractor’s Paycheck.

I am a board certified Chiropractor, and I am taking a stand against all of the sham doctors out there that continue to give the chiropractic profession a bad name.  I get so tired of hearing writers on behalf of the AMA bash this life changing profession because the majority of chiropractors are practicing a style of care that is outdated. I am afflicted with a very scientific, left handed, analytical brain that will not let me believe in wish-washy crystals and body energy based therapies, nevertheless, expect them to work. I focus on scientific, reproducible, evidence-based, and clinically proven methods of patient care to ensure that each patient that walks in my door will get quality care that they can depend on. Each patient that enters New-Start Health Center will know their exact pain generator, they will know how long my treatment plan is supposed to take to alleviate their pain, and they will know that I am not going to drag them through an exhaustive examination, expose them to unnecessary radiation, or give them a 6 month, 40 visit, treatment plan that “might” help.

I love my profession, and I love what it can do for patients, but since the birth of chiropractic about 110 years ago, things have changed. DD Palmer made the first chiropractic adjustment when he adjusted a deaf janitor named Harvey and his hearing came back. Unfortunately, in the majority of the chiropractic curriculum, these are the stories that you hear. They teach the students how chiropractic adjustments are the only therapy in the world that can put the spine back in alignment and turn the power of the nervous system back on to restore health. I remember stories of how adjusting the right ankle of someone with hemorrhoids has made them disappear. Adjustment of a patient’s big toe made his back pain go away. We were trained that each spinal level has a set of nerves that innervate each organ, tissue, and cell in the body, and if they spine is out of alignment, the body cannot heal itself. All of these concepts were so inspiring to hear, I, Me, could heal the entire world with any condition, with my hands.

Unfortunately, science happens, and the medical world demands you to prove that you can treat a condition. Considering the facts, you will not find any clinical studies that prove that any bone is displaced in the body, and that bone puts pressure on spinal nerves. You will not find reproducible evidence that adjusting the first vertebrate of anyone will lower blood pressure, or adjusting the lumbar spine in a child will cure bed wetting.  You can find case studies and hear stories of these miraculous events, but again, they are not scientifically proven, or even suggestible that they will work. It is only a trial and error method. A treat and hope plan. NOT GOOD ENOUGH FOR ME.

Where you will find scientific evidence, case studies, control trials, and even validity within the medical world is this. The chiropractic profession is proven, time and again, to be more effective at treating acute back pain and keeping it at bay, than medication and surgery.  That is what the profession has settled into. We are PAIN MANAGEMENT doctors. The adjustment is effective at blocking the pain receptors. We have a superior education to any other primary care physicians in the realm of the musculoskeletal system. Considering low back pain is one of the top reasons Americans seek out medical relief, I would say we have settled into a pretty good niche. Chiropractic physicians need to understand that we are primary care physicians, we are capable of diagnosing any major condition, but we are not treating these conditions with adjustments, there just isn’t enough proof to suggest it.

You should NEVER hear at a doctor’s office, “this might work” or “this has worked before.”  If you are wondering why you have been to your chiropractor for so many visits and they never seem to address what is really going wrong with you, then there is a good chance you have become a Chiropractor’s Paycheck instead of a Chiropractic Patient. 

 Here are some quick tips to analyze if you have been seeing a sham doc instead of a real doc.

1.       X-ray every patient on their first visit.

 The traditional chiropractor loves the use of x-rays to tell patients things that will scare them, and ensure each of them that ongoing care for the rest of their life is necessary.  Chiropractors love showing the patient the film, too. This is where a trained eye can point at anything and tell a layman person whatever they want, and the patient has no choice but to believe them because odds are, they have no clue what the doctor is pointing at.  The basic concept I want you to take from this is that you cannot see any pain generators on an x-ray that were not determined from the history and physical examination.

If you present to a chiropractor with a new ache or pain and you haven’t suffered from any trauma, make sure you ask if an x-ray is necessary and ask the doctor what they are looking for with this film. If they say something like, “we are looking to see how healthy your spine is,” run. You should not expose yourself to the radiation if you can avoid it.
The common findings on x-rays that I am sure you have heard are:

  Reduced curvature in your neck
I am sorry, but there is no evidence that displays how a reduced curvature in your neck will lead to a pain syndrome. Better idea, why not just do a physical examination and check the patient’s posture. I can bet that if a patient has a forward head translation in regard to their shoulders, they have a reduced lordotic curvature in their neck.

 Degenerative Disc Disease
“We cannot cure this in your spine, but we can make sure it doesn’t get worse.” I can understand why every chiropractor wants to tell a patient they have this. It is SCARY. The irony is, general osteoarthritis, Degenerative Disc, Degenerative Joint Disease, or whatever you want to call it, is NOT an inflammatory process within the body. This means, there is no pain pattern that is clinically reproducible with this diagnosis.

2.       They diagnose you with a SUBLUXATION.
The principles of chiropractic are based around this concept. A bone in the spine gets out of alignment and it puts pressure on the vital nerves coming from your central nervous system, reducing nerve flow to the peripheral tissue.
 
As I stated before, it all sounds good and believable, but scientific evidence says otherwise.
If you even hear your chiropractor use the term subluxation in his report of findings, RUN.

3.       40+ visits treatment plan
Have you ever heard your friend say, “Once you go to a chiropractor you have to go forever!”
This is my favorite. The profession is always thinking of ways to maintain patient compliance. So they have designed a generic treatment plan that will cure your ails.

3 visits/week for 3 months, then 2 visits/week for 1 month, then 1 visit per week for 6 months, then 1 visit/month FOREVER.

News Flash, if a chiropractor does not make a considerable difference in your symptoms in 3 weeks, chances are, they, WON’T.

4.       Same treatment for every conditionI have shadowed some doctors in my educational pursuit. I kept questioning whether I was crazy or the doc was. It did not seem to matter what the patient was coming to see that doctor for. Headaches, back pain, neck pain, dizziness, vertigo, digestive issues, bed wetting, pregnancy, You NAME IT, the doctor always did the exact same treatment. They would start face up, get their neck adjusted, on both sides, then rolled up into a ball and their mid back adjusted in about 3 places, then the doc would roll them on each side and adjust both of their hips or low backs, (pretty much whatever they could get to make the popping sound), then EVERY patient went to the next room where they would either get electric stimulation and heat or the traction table.

Either way, it did not matter what the patient was suffering from, they all got the exact same treatment protocol.

Rule of thumb, if you do not find your treatment plan to be focused on your area of complaint, or if you do not find your treatment options progressing through the treatment plan, RUN.

Of course, we all like the feel goods that go with a traction table or a heating pad, but you are not at the chiropractor for this fluff, you are there to get treated.

5.       “Wellness” Center
I constantly ask my colleagues that name their business, Bob’s Chiropractic and Wellness or Smith’s Chiropractic Wellness Center, “How exactly are you promoting “Wellness”?” The responses are always entertaining. Some discuss the obvious chiropractic mentality, “Alleviating the subluxation in the spine will allow the body’s natural healing powers to make every cell in the body well and happy.” Some reply with a better answer, “We always promote a healthy lifestyle in the office.”

Wellness can only come from one thing, good health. Do not let any chiropractor tell you that your spine is out of alignment and this is why you are getting the flu. Wellness comes from a low stress life that encompasses a healthy diet and some moderate levels of physical activity. If you think that you are going to have “Wellness” by getting adjusted after eating a McDonald’s Big Mac and fries, sorry, you are sadly mistaken.
 

There is always a principle that I follow, To Each Their Own. Maybe you have experienced all of the five things and think that I am crazy for saying it. You might love the way you can go into your chiropractor’s office and get treated the same way you always have. Maybe you get great relief from this style of therapy, and maybe you have been one of those miracle cases that demonstrate that a chiropractic adjustment can do more than just block pain.

I am just trying to help the stability in my profession by explaining the true evidence behind our profession. Chiropractic was created by a genius of a man that was 70 years ahead of his time. He was designing adjustment protocols and treatment strategies long before they were even understood. But this is now 2014, we have the Internet, and anyone in the world that googles “subluxation” is probably going to think that their chiropractor is out of his mind.
If you have heard your doctor say, “you are suffering from a subluxation, and it is putting pressure on your nerves, keeping your body from homeostasis,” RUN.

It is scientific research and evidence that shows how effective a chiropractic treatment plan can be for managing any musculoskeletal pain syndromes. If you are seeking relief from any aches or pains, be sure to find a chiropractor that is geared to take on the case. Make sure you get into an office that is primarily working to get your out of pain so you can better enjoy your daily life, and stop wasting money on the flying 7 adjustment and some electric stimulation.

CVS, You Tried, You Failed.

I heard the breaking new this morning and started laughing.

CVS is going to stop selling tobacco products.


I guess I am missing the point.

 Are the CVS stores trying to minimize its sales of poison?

Are they not interested in selling something that is designed to destroy your health?

As soon as I got to my office, I started to do some research on the matter, and it again, makes me laugh.


CEO Larry Merlo noted that chronic conditions are made worse by smoking.
‘‘We've come to the conclusion that cigarettes have no place in a setting where health care is being delivered,’’ he said

So, I assume that Larry seems to think that health care means pharmaceutical medications?

Lets start with the facts. The CDC has examined the death toll of tobacco use and has come up with a wild number of about 480,000 deaths a year are attributed to the use of tobacco. 

Of these 480,000 deaths, the breakdown of causes seem to be a bit disorganized as to how they link them to smoking of cigarettes. We all know that smoking cigarettes is linked to lung cancer. This makes sense. Cancer is related to stress on a function of the body, and smoking puts stress on the lungs causing the cells to mutate and become cancerous. 

We have all seen pictures of what lungs look like in a smoker instead of a non smoker.

We all know the risks of smoking and how harmful it is for the body. 

Anyone that smokes cigarettes is not sold something that is supposed to benefit their health. Smokers make a conscious decision to light up and harm their health.

The second leading killer due to cigarette smoking is heart disease?

I have done extensive research on the inflammatory condition called heart disease.

According to The National Heart, Lung, and Blood Institute, "Smoking is a major risk factor for heart disease. When combined with other risk factors—such as unhealthy blood cholesterol levels, high blood pressure, and overweight or obesity—smoking further raises the risk of heart disease."

So, to be clear, is smoking the risk factor, or does smoking raise the risk of. I am not sure, but I do know this, You will not develop heart disease unless you eat more sugar than you are supposed to. Arteriosclerosis is caused by inflammation in the arteries. Sugar is the cause of elevated LDL cholesterol, the cause of increased insulin in the blood, and increased fat tissue in the body. 

What is the major risk factor for heart disease?

The VAST MAJORITY of HEART DISEASE PATIENTS are OVERWEIGHT.

Some underweight people do get heart disease. They could suffer from idiopathic elevated cholesterol. They can suffer from undiagnosed Type 1 Diabetes.

The other factor is that ALL diabetic patients that go through the health care system, and take the "medications" for Type 2 Diabetes will develop and die from heart disease. That is a fact.

What causes people to be overweight?

HMMM, maybe CVS should rethink the long isle of candy, sugary treats, soda, potato chips, and alcohol.

The real cause of heart disease lays within our diet and lifestyle, maybe CVS should think about changing into a Whole Food Store? Instead of a great place to get discount candy after Halloween or any other holiday?

The real story here, is that smoking is bad. Everyone knows that. No one has ever tried to sell someone a cigarette with the explanation that it will help your health. 

So, CVS is stopping selling these products because it is known that they do not help your health, and a place that is associated with the the health care system does not want to promote a product that hijacks your health?

Again, CVS sees that assisting the medical system in passing or dealing their drugs to a consumer that have been prescribed by a "health" care professional is what they should be doing to promote "health."

What are the facts within the heath care system?

I have some alarming statistics that will help you understand my confusion as to why CVS thinks that stopping selling a product that everyone knows is bad for your health and continues to sell a product that is praised as the only way to better your health. The problem is, the medications your buy from CVS have minimal research, and typically, botched research to prove that it will help you at all.

These stats are not the most current, because the FDA and the Pharmaceutical industry realized how bad they are for business. This is what our health care system is all about, right? Business?

  • In an average year, 1.6 million people are hospitalized due to side effects of prescription drugs, and 160,000 will die. (1992)
  • In America, we consume about 15 tons of aspirin a day, or 19 billion tablets per year. Aspirin is considered harmless, right? 
    • A single aspirin will be responsible for 1500-2000 deaths/years
    • leading cause of kidney disease
    • Create ulcers, headaches, and Reye's Syndrome.
  • 61,000 Americans will suffer from prescription drug induced Parkinson's Disease a year.
  • Americans over the age of 60, over 1 million will have adverse reactions to medications.
    • 243,000 will be hospitalized
    • 163,000 will suffer from permanent memory loss
  • 32,000 elderly Americans will suffer from hip fractures due to drug induced falls.
    • 1500 will die.
  • 196,000 Americans will die from PROPERLY PRESCRIBED DRUGS.
  • Health Care costs have reached $15 Trillion dollars and no help is in sight.
  • Drug mishaps cost Americans about $182 million dollars a year, kill as many as 198,000, hospitalize 8.8 million and account for over 25% of all hospital admissions.
Do I even need to discuss VIOXX? It was a medication that was not researched with long term studies to decide its side effects and it resulted in 500,000 deaths. Half a million deaths occurred due to this medically prescribed medication. Health Care?

I guess I have a different definition of "health" care than that of CVS and the medical industry. 

I am congratulating CVS on this change. Not promoting a poison is a positive advance in our health status in America. I am just afraid they are trying to put out the raging forest fire with canteens, while their biggest money making industry are fueling it with gasoline.

Wait, did CVS forget that they sell alcohol? Doesn't it seem a little hypocritical to stop selling cigarettes but continue to sell a full shelf of alcohol products?

So, maybe this is all business?

Cigarette sales being stopped will cost CVS $2 billion dollars this year. They represent only 4% of total tobacco sales in the country. 

Will this make any real impact on decreasing the amount of smokers? I don't think so. The vast majority of patrons in any pharmacy are those of the elderly population, considering the average elderly person takes 15 different prescriptions a day. 

What are the amount of cigarettes being smoked by this population?

As you can see. Only 7.5% of the elderly population smoke. More importantly, the total smokers in America are declining. 

Smoking as a whole is declining, yet health care costs continue to rise?

Again, CVS, stop trying to put a forest fire out with a canteen. You have to keep the $50 billion dollars in advertising from big pharma from happening to make any impact on our health.

Diagnosis, The most important factor in health care

New-Start Health Center is changing the way we look at health in America.

Why is our health care system failing to improve the health of America? Why is the obesity rate climbing? Why are the prevalence of type 2 diabetes, heart disease, dementia, and all other inflammatory diseases climbing? The answer to the questions is this, misdiagnosing the true cause of each condition.  We MUST CHANGE  the way our health care system looks at the body, the way it diagnoses a condition, and the way it looks at treating a disease.

It is not the doctor’s fault. Each doctor is trained to treat each condition with a certain protocol and they are just doing their job. Antacids for heartburn, antibiotics for sore throat, tranquilizers for restlessness, statin drugs for high cholesterol, etc. are very common protocols. These therapies are effective at treating the symptoms of a bodily dysfunction, but they are missing the big picture, treat the dysfunction, not the symptoms. The main problem with treating symptoms is that it can lead to a domino effect. One medication could have a side effect, and without noticing the doctor will most likely prescribe another medication to treat the side effect caused by the first medication.

Instead of this therapy, the health professional needs to realize that the human body is just a machine. Each engine needs a few basic things to run; air, fuel, and electricity. The human body is the same, we need food for fuel, air to breath, and our nervous system is an electrical component that makes it all work. To further critique this, think about putting watered down gasoline in your car, or putting diesel fuel in a gasoline engine. What would happen? The engine might run, but it will be running sluggish, rough, and unable to perform the duties that we rely on. The human body is the same, if we put unhealthy fuel into our body, the machine will not function properly.

While studying pathology in school, I realized that I started diagnosing as a child. Growing up on a farm allowed my brothers and me to get our hands on many different types of machinery.  It was important to realize each component and function of each machine. Each time we noticed some dysfunction in a machine, (tractor running rough, tillage equipment leaving rough segments in the soil) we had to diagnose what was actually going wrong with the machine. The approach I was taught was to first know what parts made up the machine, what each part’s function was, and most importantly, know what symptom the machine would experience if that part was faulty.  Once you understand symptomology, it is important that you are not trying to just fix the symptom, but you are actually trying to alleviate the symptom by fixing the component causing it.  When you do this, you will see that the expense of the fix is much less, because you are only the one part that you need.

To translate this back to health care is simple. As a health care provider, it is essential to know the many different organs and functions of the human body. It’s important to know the complex interactions between vitamins, minerals, and hormones of the body. And again, the most important information to know is the symptoms that the body produces to let you know something has gone awry.  To properly diagnose a condition, we must read to symptoms, and link them to the true organic cause. If we, as a profession, can accurately do this, care becomes simple, health becomes evident, and dysfunction becomes non-existent.
